New Clients

Once you schedule an appointment with us, we will send you new client forms through our secure online patient portal. Please take your time to read and complete these materials. We ask that you fill out the New Client Registration Form, the New Client Lifestyle Questionnaire and the Multiple System Questionnaire and submit these prior to your first appointment with us.

If you have any prior lab work or medical records, please bring in a copy to your initial visit for Dr. Michael to review. You may also have your records faxed to us in advance at 919-495-6686 or upload your medical records to your secure patient portal.

Insurance Coverage

Our office does not bill your insurance directly, but we will provide you with a coded receipt for services, which you may submit for reimbursement to your insurance carrier. Dr. Michael is an out-of-network provider for all private insurance plans and is not a participating provider with Medicare. The amount of reimbursement you receive depends on your insurance plan’s reimbursement policy for out-of -network providers, your deductibles, copays and co-insurance. You may wish to check with your insurance plan regarding out-of-network coverage of services prior to your visit with us.

Services associated with our clinic including physician visits, counseling, supplements and laboratory testing can be submitted under your health savings account (HSA) or Healthcare Reimbursement Account (HRA).

Fees & Payment

Renovo Natural Health operates as a self-pay clinic. We accept cash, checks or credit cards for services rendered. Payment is expected at time of service.

Cancellation Policy

If it is necessary for you to cancel your appointment, we request that you call the office at least 48 hours in advance.

To cancel an appointment, please call 919-986-9940.. You may leave a detailed message on the voice mail if you are unable to speak to us directly.

Late Cancellations or No Show

We understand that unexpected changes of schedule occur and allow for one missed appointment per year. After that, patients failing to cancel their scheduled appointments at least 48 hours in advance will be billed an administrative fee.
